Heritage Home Health, Inc. provides home health care to homebound persons in South Palm Beach County, under a plan ordered by a physician. Heritage Home Health is also contracted with Av-Med Health Plan to provide home health care services to Av-Med participants.

How To Choose A Home Health Care Agency

The following are a few questions to help guide you in determining what is most
important to you and your needs.

Is the agency Medicare-certified?
Medicare certification does not reflect the quality of care of the agency. Rather, this certification allows the agency to provide Medicare coverage under strict federal regulations and guidelines.

Does the agency hold state licensure?
In Florida, a home care agency must be licensed by the Agency for Health Care
Administration (AHCA).

Is the agency accredited?

Accreditation is a voluntary process conducted by nonprofit professional organizations that determine through surveys whether the agency meets established standards.  The Joint Commission is one such organization.  Be sure to ask to what degree their surveys are “deficiency-free.”

Does the agency provide important information about services and policies in writing?
You should be able to review written statements describing services, eligibility
requirements, fees, funding sources, patient rights, complaint procedures and emergency
arrangements.

How long has the agency been operating?
While it is not always an indicator, an agency with a long history of service typically has
a positive reputation.

When does the agency provide services?

Is home health service available on the same day as referral? Can you receive services
twice daily? How many days per week. Does the agency operate on holidays? Are
services provided 24 hours per day/365 days per year?

Does the agency provide linkages?
In addition to Medicare and insurance, what links can the agency provide for additional
services or assistance in Palm Beach County?

How is a plan of care developed?
Is it based upon the patient's needs or is it strictly limited by the funding the agency receives from Medicare?
