Heritage Home Health, Inc. was founded in 1993 as the home health care agency partner of Heritage Park Retirement Communities
in Delray Beach, Florida. Today, our highly qualified staff provide
home-based skilled nursing, medical rehabilitation and private duty
services to clients in the South Palm Beach County area. Heritage Home
Health, Inc. is Medicare-certified and accredited by The Joint
Commission.
The

Peace of Mind
Heritage Home Health carefully selects all staff members to
ensure that we are providing all clients with responsive,
compassionate and high quality care. For further security, we do
background checks, verify all references and perform drug
screenings at time of hire. Our staff members are also covered
by our liability insurance policy. |
Individual Consultation
Our professionals work with every client and his or her physician in
the design of individual plans of care. We will provide quality
care givers for your needs and will answer all patient and family questions.
Our goal is to assist all whom we serve in recovery and
rehabilitation – teaching the skills that ensure good health and
wellness. (If the event a full recovery is not possible, our staff
will strive to create a comforotable and peaceful setting.)

individual with concerns about care and safety provided by Heritage
Home Health, Inc. that has not been addressed by the agency, is
encouraged to contact the agency’s Administrator. If the concerns
cannot be resolved through Heritage Home Health, the individual is
